You don't say whose adapter & patch lead you're using, but I would say a call to the manufacturer might be the best option, especially if you've followed their guidance as to which is the correct combination to use.

Personally I've used the adapters and leads from a company called Connects2 that I've always found to be good (http://www.connects2.co.uk/), so this might be another option if it's not their products that you're using now.
